CHIP 2006 SUMMIT


Who Should Attend: Veteran CHIP leaders, potential CHIP leaders, CHIP team members, conference health ministry directors, health professionals: physicians, nurses, dietitians, dentists, pastors, anyone with passion for health ministry. Friends and spouses are welcome.

subsidized by the Northern California Conference and business friends of CHIP. Meal reservations must be made by Thursday, November 9, 2006.

Refund Policy: You may request a refund minus a $25 processing fee through November 9, 2006. After this date, you may send someone to attend in your place.  The person replacing your registration must present a letter on-site that clearly lists the registrants name and the name of the person replacing them.

Special Note: CEUs (6 hours on Friday) are available for RN's. Pre-registration is required. Please call (925) 685-4300 ext. 1273.

Additional Material: For Basic CHIP Leadership Workshop Attendees only: One copy of the (300-page) CHIP Operations & Resource Manual is required per local church represented in the workshop. Cost is US$85 if prepaid by November 1, 2006 (call 580-622-3020 with credit card) or may be purchased onsite for US$95.

Sales by CHIP-Friendly Vendors: Attendees may wish to purchase additional materials at the Summit so come prepared.

Lodging: You are responsible for making your own lodging arrangements. Radisson Hotel Sacramento, 500 Leisure Lane, Sacramento, CA 95815. Phone (800) 333-3333 or (916) 922-2020. Please mention "CHIP Summit" to get the $95 rate for one to four persons including all taxes and fees. The Special CHIP Summit rates will be offered two days before and after the Summit based on availability. Reservations for this special rate must be made by November 1, 2006 to
insure availability.

Meals: You will receive noon dinners Thursday through Sabbath with your paid event registration. You may purchase optional meal packages as listed above. Meal reservations must be made by Thursday, November 9, 2006.

On-site Registration and Check-in: Onsite registration is available at the full fee - but without noon meals being guaranteed. CHIP staff will be available to help you on Wednesday, November 15 from 2:00-6:00 p.m., on Thursday, November 16, 2006 from 8:00 a.m.-7:00 p.m. or on Friday, November 17, 2006 from 8:00 a.m.-3:00 p.m. You may pay the CHIP registration fee with cash,
credit cards or checks or Money Orders made out to the Adventist CHIP Association. Meal reservations must be made by Thursday, November 9, 2006. Hotel reservations can be made after Wednesday, November 1, 2006 only as space is available.

Children: You are welcome to bring your children under your supervision. You must call the Adventist CHIP Association at (580) 622-3020 (Central Time) to register children (3-10) for meals. No special programs have been planned for children at this event.

Dress: Casual, with Dressy Casual on Sabbath. Please be prepared for possible cool outdoor weather.
